If you are Dentist or Dental Specialist who has received a Practising Certificate application but will not be practising in New Zealand
during the period 1 October 2011 - 30 September 2012, and you wish to be retained on the Register, click

Dental Hygienist, Dental Therapist, Orthodontic Auxiliary, Dental Technician or Clinical Dental Technician
If you are Dental Hygienist, Dental Therapist, Orthodontic Auxiliary, Dental Technician or Clinical Dental Technician
who has received an Annual Practising Certificate application but will not be practising in New Zealand during the
period 1 April 2011 - 31 March 2012, and you wish to be retained on the Register, click here for an Application to be Retained on the Register.
After printing and completing the relevant form above, please discard the Annual Practising Certificate Application
form you have been sent, and only return the completed Application to be Retained on the Register form and fees to the Dental Council
for processing.
