Registration - 2009 NZ Graduates
If you are a New Zealand graduate registering for the first time please complete the appropriate application form and send all relevant documents and fee payment to the Dental Council.

CHECKLIST

  • Completed application form for registration including signature
  • Current hepatitis B and C laboratory report (must be less than 6 months old)
  • Fee payment
  • Passport sized photo
  • Certified copies from the relevant identification pages of your passport (overseas citizens) or drivers license (NZ citizens only).

NOTES

Identification:

Please note that when an identification document (passport or drivers license) is provided, Council requires that the copy of the document is certified by an appropriate person. A person eligible to "certify" documents in New Zealand is a person who is qualified to take oaths under the Oaths and Declaration Act 1957 which primarily includes Justice of the Peace, Barristers or Solicitors and Court Registrars.

Declarations:

If you declare on your application form that you have been subject to any of the following:
  • criminal conviction
  • mental or physical condition
  • professional disciplinary proceedings
Council has the right to request a police certificate, medical report, certificate of good conduct or such other information as it may reasonably require.

Registration prior to 25 December 2009 :

If you require registration prior to 25 December 2009 your registration application form will need to be received at the DCNZ secretariat office by 18 December 2009.

Timeline:

Submission of a completed application form with all documents and payment attached will be processed in 5 working days.

Expiry:

The Graduate application form expires on 31 March 2010.
