Address and Name Changes
Registered oral health practitioners are legally required to inform the Council of name and address changes.
Address Changes
Under the HPCA Act, you are required to inform the Council of your current postal address, residential address,
(and if applicable work address) and to promptly inform Council of any changes of address (Sn 140).
If you do not provide the Council with a current address you risk removal from the Register.
You can print and complete the following form and mail or fax to the Dental Council to advise of Changes of
Address (either Postal or Practice Address):

Name Changes
Requests for name changes need to be accompanied by a certified copy of the relevant document e.g. marriage
certificate.
