Dentist or Dental Specialist
If you are Dentist or Dental Specialist who has received an Application to be Retained on the Register but wish
to practise in New Zealand during the period 1 October 2011 - 30 September 2012, click here
for an Application for a Practising Certificate.
Dental Hygienist, Dental Therapist, Orthodontic Auxiliary, Dental Technician or Clinical Dental Technician
If you are Dental Hygienist, Dental Therapist, Orthodontic Auxiliary, Dental Technician or Clinical Dental Technician
who has received an Application to be Retained on the Register but wish to practise in New Zealand during the period
1 April 2011 - 31 March 2012, Click here for an Application for an Annual Practising Certificate.
After printing and completing the relevant form above, please discard the Application to be Retained on the Register
form you have been sent, and only return the completed Application for an Annual Practising Certificate form and fee to the Dental
Council for processing.
